Immediately, our professional, very experienced RN, MSN, Case Manager, will conduct a needs assessment by phone and if necessary in person. The goal of this needs assessment is to get a better understanding of each person's unique needs. We assess the following:
- Medical condition
- Physical needs
- Emotional needs
- Preferred social environmen
